Terms of Service

DentalIQ is an AI Practice Intelligence platform for independent dental practices, operated by DentalIQ LLC, a Wyoming limited liability company (EIN: 41-5101841). Our platform consists of 11 production modules — InsureIQ, AcceptIQ, NoteIQ, ReviewIQ, ReportIQ, LabIQ, ProcureIQ, PrescriptionIQ, RecallIQ, BillingIQ, and AuditIQ — covering insurance verification, treatment acceptance, clinical documentation, review management, monthly reporting, lab case tracking, supply intelligence, post-care patient outreach, patient recall, billing leakage recovery, and practice audit scoring. DentalIQ is also available as a mobile app on the Apple App Store and Google Play.

DentalIQ offers the following subscription tiers, billed monthly via Stripe:

• Platform ($599/month): Includes InsureIQ, AcceptIQ, NoteIQ, ReviewIQ, ReportIQ, LabIQ, ProcureIQ, AuditIQ, and the Practice Intelligence Dashboard.
• Platform + RecallIQ ($799/month + 10% recovery fee): Includes everything in Platform, plus RecallIQ (patient recall: who is due, a call list, and AI-drafted texts and emails that your team reviews and sends) and PrescriptionIQ (post-care AI outreach). A 10% fee applies to revenue recovered through RecallIQ patient reactivation.
• Practice OS ($999/month): Includes everything in Platform + RecallIQ, plus BillingIQ — a 7-tool revenue intelligence suite covering Claim Scrubber, Narrative AI, AR Aging Dashboard, Ledger Audit, Denial Appeal Generator, PPO Fee Schedule Analyzer, and Billing Health Score.

All subscriptions include access to the DentalIQ mobile app (iOS and Android) at no additional charge. You may cancel at any time through the customer portal. Cancellation takes effect at the end of the current billing period.

On plans that include RecallIQ, the practice pays DentalIQ 10% of the revenue from a recovered patient's visit, but only when RecallIQ reached the patient first: a RecallIQ message or a call logged in RecallIQ in the 90 days before the visit. A patient who comes back with no RecallIQ contact carries no fee.

• Amount: 10% of the visit's paid claim (insurance paid plus the patient's portion) when DentalIQ has one; otherwise 10% of the amount the practice entered when marking the patient recovered.
• Billing: each month's fees are listed in RecallIQ under Recovery fees and added to the practice's next DentalIQ invoice, on or after the 5th of the following month, and charged with it.
• Disputes: until a fee is billed, the practice can dispute it in RecallIQ with a reason; it is then held until DentalIQ reviews it. For a fee already billed, contact [email protected] and we will credit it if it is wrong.

DentalIQ offers the following onboarding options:

• Self-Onboard (Free): Downloadable setup guide, in-app onboarding flow, and video walkthroughs.
• Remote Onboarding ($400, one-time): 1-hour Zoom session with the DentalIQ team, including data import assistance and staff training.
• On-Site White Glove ($2,500, one-time): Full-day on-site setup, data migration, and staff training at your practice location.

Onboarding fees are non-refundable once the session has been completed.

New accounts receive a 14-day free trial with access to all Platform features. No credit card is required during the trial. At the end of the trial period, access to the platform will be restricted until a subscription plan is selected.

DentalIQ Mobile is available on the Apple App Store and Google Play and is included with all subscription tiers at no additional charge. The mobile app requires an active DentalIQ subscription to access full features. A demo mode is available within the app for evaluation purposes and provides access to features using sample data without requiring an account.

Use of the mobile app is subject to these Terms of Service, the terms of the store you downloaded it from, and our Privacy Policy.

You agree to use DentalIQ only for lawful purposes in connection with the operation of a licensed dental practice. You may not:

• Share login credentials with individuals outside your practice
• Use the platform to process data for practices other than your own without written authorization
• Attempt to reverse engineer, scrape, or extract data from the platform
• Use AI-generated content from DentalIQ as a substitute for professional clinical judgment

DentalIQ processes patient information on behalf of dental practices. Which of our service providers have signed a Business Associate Agreement, and which have not yet, is listed in section 5 of our Privacy Policy; a practice that needs a Business Associate Agreement with DentalIQ can contact us before loading patient data. By using DentalIQ, you represent that your practice has the authority to share patient information with our platform for the purpose of operating your practice workflows.

DentalIQ does not sell patient data. Patient information is used solely to provide the services described in these Terms.

Program: DentalIQ patient texts. Dental practices that use DentalIQ text their own patients from the DentalIQ number, (870) 770-3832, with reminders that a routine checkup is due and short follow-ups after a visit (which may include a link to the practice's review page). Every text names the practice it is from.

Consent: patients agree at their dental practice, for example on the practice's patient form, as shown at dentaliq.app/sms-consent. The practice records the consent in DentalIQ, and DentalIQ sends no texts to a patient without it. Consent is not a condition of treatment.

• Frequency: varies, typically a few messages a year.
• Cost: message and data rates may apply.
• Opt out: reply STOP (or STOPALL, UNSUBSCRIBE, CANCEL, END, QUIT) to any text. You will get one confirmation and no further texts from this number. Reply START to opt back in.
• Help: reply HELP, call your dental practice, or email [email protected].
• Carriers are not liable for delayed or undelivered messages.

Practices agree to text only patients who gave consent, to record that consent in DentalIQ, and to honor opt-outs (DentalIQ honors STOP automatically). Phone numbers and consent are handled as described in our Privacy Policy.

DentalIQ uses Anthropic's Claude models (run on Amazon Bedrock) to generate clinical notes, treatment plan summaries, outreach messages, lab case quality assessments, billing narratives, and practice reports. AI-generated content is provided as a starting point and must be reviewed and approved by a licensed dental professional before clinical use. Nothing AI-generated is sent to a patient until a person at the practice reviews and sends it. DentalIQ is not responsible for clinical, financial, or operational decisions made based on AI-generated content.

DentalIQ LLC owns all rights to the DentalIQ platform and mobile app, including its software, design, AI prompts, and documentation. DentalIQ™ is a trademark of DentalIQ LLC. You retain ownership of your practice data. By using DentalIQ, you grant us a limited license to process your data to provide the services.

To the maximum extent permitted by law, DentalIQ's liability to you for any claim arising from your use of the platform or mobile app shall not exceed the total fees paid by you in the three months preceding the claim. DentalIQ is not liable for indirect, incidental, consequential, or punitive damages.

Either party may terminate the subscription at any time. DentalIQ reserves the right to suspend or terminate accounts that violate these Terms. Upon termination, you may request an export of your practice data within 30 days.

These Terms are governed by the laws of the State of Wyoming. Any disputes shall be resolved in the courts of Sheridan County, Wyoming.

We may update these Terms from time to time. We will notify you of material changes via email to your account address. Continued use of DentalIQ after changes take effect constitutes acceptance of the updated Terms.
